Job Description:

This is a community-based position that supports the healthy development and well-being of children and families. Through one-on-one home visits, individual will focus on assessing family needs, educating and supporting parents, and connecting families to additional services, with the aim of improving maternal-child health, child maltreatment prevention, and family economic self-sufficiency.   

Duites:

  • Work closely with other professionals and community partners to facilitate referrals, coordinate services and support for families, and conduct follow-up activities on behalf of families.
  • Engagement in active and ongoing recruitment of home visiting families (door-to-door recruitment, program check-ins, etc.).
  • Maintain a caseload of a minimum of 15 families at any given time.
  • Conduct home visits regularly with enrolled home visiting families to assess needs, provide guidance, and model effective parenting techniques. Utilize evidence-based models and curricula to implement services from prenatal to 3 years of age.
  • Connect families with community services and assist families in navigating resources effectively.
  • Maintain accurate records of visits, assessments, and family progress. Submit timely reports through home visiting database, and ensure confidentiality is upheld.
  • Engages in program activities (staff meetings, supervision sessions, trainings etc.) for the purposes of providing holistic care and increasing maternal and early childhood outcomes for families.
  • Plans and facilitates program socializations and ensures complete, accurate and timely records (flyers, purchase orders, feedback forms, sign in sheets, etc.) are submitted for each event.
  • Conducts and reviews child developmental and adult wellness screenings, and routes referrals to appropriate entities for additional resources and supports.
  • Participates in monitoring, data collection, and continuous quality improvement for the purposes of assessing and evaluating the program to meet grant requirements, ensuring quality standards of care, and meeting the desired outcomes of maternal-child health.
  • Attends trainings to become certified in curricula including but not limited to: Family Spirit, Certified Peer Support Work, Circle of Security, T.R.I.B.E.S., etc. Skilled in the administration of screenings including but not limited to: ASQ, PICCOLO, PHQ-9, CAGE-AID, HITS, etc.

Additional Responsibilities:

  • Prepares and disseminates program marketing materials and prevention and awareness resources.  
  • Comes prepared to meetings to offer input and feedback about services and home visiting implementation strategies.

Qualifications:

Education:Associates or Art (AA) degree in Early Childhood Education or related field including Social Work, Family Studies, Child Development is preferred.

Experience/Intermediate Knowledge: Understanding of adult and child learning theory such as behavioral objectives, methodologies; at least 2 – 4 years experience working in early childhood setting; understands and can relay information in Keres language; awareness of cultural parenting practices and has ability to communicate information; 

Required Licenses/Certifications: Valid driver’s license.

Physical Requirements: Must have manual dexterity to enter and retrieve data from a computer. Ability to sit and stand for long periods. Bends and stoops regularly.

Preferences: San Felipe Preference and Veteran’s Preference.

Working Conditions:

Works in office. Sits for long periods of time and types.
