About the Role

We are seeking a skilled and compassionate Occupational Therapist (OTR) to join our Home Health team . This is an exciting opportunity for a motivated professional to make a meaningful impact by helping patients regain independence in their own homes.

Key Responsibilities

• Provide in-home occupational therapy services to patients.
• Develop and implement individualized treatment plans to improve patients’ functional abilities.
• Educate patients and caregivers on therapeutic techniques and home safety.
• Document patient progress and maintain accurate clinical records.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Qualifications

• Active Occupational Therapist (OTR) license in Michigan.
• Some experience in Home Health or related setting preferred.
• Strong communication and organizational skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation for home visits.
