Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Schedule: Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
Occasional evening/weekend overtime available
Field-based position with travel required (mileage reimbursed)
Active CNA certification required
Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands-on care and support, including:
Assisting with mobility, transfers, and ambulation
Providing personal care (bathing, grooming, dressing, feeding)
Offering companionship and emotional support
Performing light housekeeping tasks
Documenting care accurately and in a timely manner
Supporting a culture of compassion, quality, and respect
You will be an integral member of our interdisciplinary team focused on clinical excellence and patient-centered care.
Active Certified Nursing Assistant (CNA) certification
Valid driver's license, reliable vehicle, and current auto insurance
Compassionate, reliable, and self-directed
Strong communication and time-management skills
Ability to work independently and follow care plans
Commitment to serving patients with chronic, serious, or terminal illnesses
